Side Reaction and its Management of the Oral Hypoglycemic Agents

We studied on 53 RH-787 inoxicated patients prospectively and analyzed 198 reported cases. The results were as follows. 1)The proporf-ian of male to female was 1 to 2.3 ranging from 1 year and 8 months to 67 years of age, and they ingested 5 to 62,5 gm of RH-787 for the suicidal purpose in the majority of patient(66.9%) 2)Clinical symptoms were nausea(36.8%), distuibance of consciousness(36,8%), abdominal pain, urinary disturbance, sensory disturbance, general weakness,and anorexia. 3) About 80% of patient showed fasting hyperglycemia or abnormal glucoae tolerence curve, and ketois were in 50% of patients. 4) Such cases as gastric lavages were performed within 15 miniutes after ingestion of RH-787 showed no hyperglycemia or abnormal gucose tolernece curve. 5)Abnormal laboratory findings such as Ieukocytosis(62.5%), elevated BUN and creatinine and proteinuria, elevated SGOT and alkaline phosphatase levels were noted, they were normalized however,following by control of glucose. 6) Cerebrospinal fluid examination showed normal ranges of pressure, color and cell counts increasd glucose (58.0%) and protein (47.7%) 7)Pathologic findings reported massive necrosis of Langerhann's island of pancreas, fatty change of river, vacuoIes and.acute necrosis in tubular epithelium in kiddney.

The complications af diabetes mellitus is well establshed as a leading cause of death, even though the insulin has contributed in great deal in cantrolling diabete., Therefore we have studied on the inicdence ef diabetic symptoms and complications among the 5.601 diabetes visited to the Diabctic. Clinic of S,N.U Hospital from the January, 1957 to June, 1977, and the followlng results were observed. The frequent symptoms were polydipsia, fatigue, neuropathy,weight loss, and impotance in sequence. 2. As the metabolic complication, diabetic ketoacidlosis, and cataract had a frequency of 1.2% and 9.9%, respectively. 3, As the vascular coirnplication.Hypetension had a frequency of 37.9%, perpheral vacular disease 3, 0%, coronary artery disease 2.2%, retinopathy 34.4% and nephorpathy 24.5% respectively. 4. As of neuropathy periphal nenropthy had a frequency of 32.7%, and autonomic neuropathy 12.8% respectively. 5. The frequency of infection was 28.0% of all diabetic, As of infections, tuberculusis had a frequency of 15.4%. 6. The retinopathy and neropathy were increased according to the duration of disease. The frequencies of the retinopathy nephropathy and infections were also increased in the groups with high blood sugar levels. 7.Cmpared to the previous results, the frequencies of hypertention, neuropathy, nephroplthy and cataract were increased.

Since 1976 when Lee first reported on rodneticide RH-787 intoxication in man,a nuimber of report. On the subjct have been madeboth in Korea and the United States. This papersums up clinical manifestation and outcome of 404 cases of RH-787 intoxication in man reported in the literature. In Korea frorn jauary 1976 through December 1978. 1. RH-787 was mostly ingested for purpooee of suicide : one- third of the patients were in the second decade of age and the female was predominant over the male in ratio of 2 to 1. 2. Neuropathy was the most common manifestation, being present in 82.8% of the patients followed by hyperglycemia (60.6%) and diabetic ketoacidosis (23.3%), 3. Peripheral neuropathy both sensory and motor, was the most common neurological manifestation, follwed by disturbance of sensorium and autonomic dysfunction. 4.The mean mortality of the patient.with RH-787 was 27% ranging from 11.3% to 83.3% apparently increasing with passage of time because of progreasive malnutrition and complications. 5. Patholoigical findlings descbribed in four patients were nercrosis of. Langerhan's istet of the pancreas and of f tubular epithelium of the kidney.

As the abnormalities of glucose metabolism are among the commonest problem in cliniccal practice, it is required to measure the blood sugar levels accuratly and rapidly without resort to laboratory assistance. Following the introductlon of Dextrostix Reflectance Melter, many reports demonstrated that Dextrostix Reflectance Melter was shown to have excellent result as were conventional laborator techniques. Recently, attempts to atililze this techniques with cerebrospinal fluid have been designed we studied the correlation between the Dextrostix Reflectance Melter method and O.T.B.(O.Tolaizine Bolate) technique based on glucosue oxidase method in the measurement of cerebrospinal fluid glucose. The result were as follows: 1. The range for cerebrospinal fluid glucose by Dextrostix Reflectometer method was 20 ~ 170 mg/dl (mean; 63.93mg/dl). 2. Dextrosticx reagent strin determinations of cerebrospinal fluid glucose by reflectometer was showen to have significant correlation with O.T.B. technique based on glucose oxidase method (r=0.9333. Y=1.05X-10.36). Therefore, it is suggested that the use of Dextrostlx Reflectance Melter may provlde a rapid and accurate estimation of cerberospinal fluid glucose.

Knowledge and attitude of diabetics toward their illness were investigated in 287 cases who were randomly sampled from our diabetic clinic by questionnaire. Following results were obtained.: Highest incidence was observed in the age group of forties, followed by fifties and thirties and male were affected more frequently than female (1.63: 1). Overweight was noted in 53% of patient; Sixty per cent had their illness less than 3 years duration and half of them experienced hospital admission. About their knowledge toward diabetes, haIf of patients were aware of it's causes, 93% it's symptoms, 70% its complications, 60.6 % urine sugar testlng, 28.2% normal blood sugar level. Especially about diabetic complications.17.1%(49 case) know about hypoglycemia and 3,5%(10 cases) about diabetic coma. And approxinsately 45% of them were well oriented in the necessity of foot care. Approximately half of the patients recognized importance of diet, but 25.8% (74 cases) know about KDA Exchange Diet Planning. One of most frequent and important points cancerning abouf ditbetic care has been brought up by these questonnaires; Folloowing diabetic diet strictly as prescrived by physician.

Recently the incidence of diabetic patients in Korea is increasing continuously and the pattern is changing in several aspects. To study the changing in pattern of diabetic incidence, authors evaluated the 5601 cases of diabetic patients, who registered as diabetic to Diabetic Clinic. Seoul National University Hospital, from 1957 to 1977. Followings are the results: 1. The incldence of diabetic patients among the patients in the Department of Internal Medicine was 1.51% and the anaual incidence analysis revealed linearly increasing pattern, and the increase was statistically significant (r=0.92. P<0.001). 2. Male to female sex ratio was 1.74 : 1, and the annual decline of sex ratio was linear and significant (r=0.67 p<0.05) 3. The distribution of diabetic patients by age is as follows : 3rd decade 5.4%, 4th decade 19.1%,5th decade 33.2%, 6th decade 28.4% and 7th decade 10.4%. But the incidence of 6th and 7th decade diabetic patients seemed to increasing in these 3~4yrs and 6th decade patients were most prevalent in these days. 4.Positive family history of diabetes was recognized in 17.1% of diabetic patients and 1% of patients were composed of secondary diabetes. Steroid abuse contributed a half of the secondary diabetes cases. 5. Eight percent of observed patients lived at the capital city and the two third of the patients were of mid to high socio-economic ststus.
